The name is NOT supposed to have any of these sounds. It's written "Фёдор" and reads [ф'одар] or [ф'одр]. No [х] ([h]) or [к] ([k]) letters or distinctive sounds whatsoever. To make sure that I'm not crazy I googled some different people saying the name, and I heard at least one clear "fy-oh-dor" (though it was not a native-russian pronuncication).
So. If someone unfamiliar with language heard someone else saying the name not clearly enough, that someone might have thought that it should be "Фёхдор" and incorrectly write it in Latin letters as "Fyokhdor".
That all seems a bit far-fetched for me, and even more so if we take into consideration that not Google nor Yahoo can find anything with the word "Fyokhdor" but are generous at "Fyodor" results.
Unless, y'know, Ohkubo suddenly decided "to be smart" and found wrong pronuncication instead of searching for how it should be written. Or he may have some "helpful" friend that provided him with wrong sound or plain wrong translit. In this case, he better should've trusted Wiki.
